Job Description
Pharm D. or B.S. in Pharmacy with 3 years experience as a Manager and 5 years of hospital experience. Must have current Texas Pharmacy Licensure, or license eligible. ASHP accredited residency is highly desirable. Responsible for clinical pharmacy services for Hospital. Developes, implements and evaluates the effectiveness of clinical pharmacy services. Evaluates therapeutic and cost effective use of medication therapy and makes recommendations for formulary considerations, policies/protocols, and individual patient therapy. Non-voting member of P&T committe, providing drug information, including,but not limited,drug monographs,results of medication use evaluations. Coordinates the adverse drug reaction monitoring program. Provides training to pharmacy staff to perform clinical activities. Serves as the drug information pharmacist including management of formulary. Full Time; 40 hrs weekly; Monday-Friday; Day Shift; 8:00AM - 4:30PM;
