Northern CA Day Shift Monday - Friday

Our client, a large home health care company in Northern CA, is adding to their Pharmacy staff. They have opportunities for 3 day shift Home Infusion Pharmacists.

As a Home Infusion Pharmacist you will work closely with nurses, rehab therapists, social workers and patients. The Pharmacist ensures the timely and accurate preparation and distribution of pharmaceutical products for clinics and home infusion programs

Responsibilities:

· Drug requests will process according to benefit guidelines · Review new drugs · Process prior authorizations, coverage determinations, grievances, appeals and exceptions · Involved with care management and disease management programs DUR · Prepare pharmacy reports

Requirements: · Current CA · BS of Pharmacy or PharmD · 1 year of home infusion experience
