100+ bed acute care Hospital has a Clinical Pharmacist opening SUMMARY OF DUTIES: The Clinical Pharmacist provides pharmaceutical care to patients at the facility and serves as a resource and a drug information specialist to medical, pharmacy, nursing staff and allied healthcare providers. The clinicians serving in this position perform comprehensive clinical assessments and collaborate with the integrated healthcare team to provide quality patient care. The Clinical Pharmacist is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients within the hospital system.

Job Description: · Responsible for administering medications based on physician orders. · Performing the day-to-day professional operations of the department which includes drug acquisition, distribution, and monitoring, identifying, resolving, and preventing drug related problems that may affect optimal patient care outcomes and to research and communicate to our medical and nursing staff, as well as to all ancillary services. · Assists Nursing and physician staff in using computerized order entry of medication orders.
