Job Description
CLINICAL PHARMACIST
Our client, one of the top providers of home respiratory products and services in the U.S. has a great opportunity for a CLINICAL PHARMACIST at their Owings Mills, MD location.
The hours are Monday - Friday from 8:30AM to 5PM, overtime as needed.
This role is responsible for the following: * Performs specified distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies. * Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy. * Communicates the results of therapeutic drug monitoring activities the physician, nursing and other appropriate persons. * Receives, verifies, reviews and documents all new patient orders. * Conducts the initial and ongoing assessment of specific patient, drug and disease process to identify pertinent drug-related problems or needs. * Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy. * Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information. * Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding medications and their use. * Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars and conferences. Maintains required Continuing Education Units. * Attends "clinical meetings" and other patient-oriented meetings with Infusion personnel. * Participates in on-call rotation. * Supervises, coordinates and /or participates in the preparation of sterile admixtures, dispenses all pharmaceuticals and related supplies and equipment in accordance with federal, state and local laws and Company P&P's.
Required Skills/Qualifications:
Proper state licensure after receiving degree and 1-3 years experience is required. Experience and strong knowledge base in intravenous admixture services and intravenous therapy. Responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals. Ability to problem solve.
