Job Description
Community Hospital located in Southern Vermont with close proximity to Albany is seeking a highly motivated individual to join our pharmacy team. The full time position includes rotating day, evening, and weekend shifts and shared holiday coverage.
The Pharmacy Services Department provides progressive services to hospitalized, sub-acute, and long term care patients. The clinical services provided by the pharmacists include pharmacokinetic monitoring, long term care chart review, unit-based activities, and overall consultative services for other healthcare providers, including hospitalists who provide on site 24 hour coverage. We utilize technology to improve medication safety and workflow efficiency including use of profile-driven, automated medication dispensing and computerized order entry that is fully integrated with lab data, nursing notes, dictated physician notes, and other important aspects of the patient medical record.
