Job Description
Clinical Pharmacist-1st shift available and 2nd shift available
The Clinical Pharmacist position employs an integrated practice model to achieve the department’s pharmaceutical care mission. Patient care activities will include, but are not limited to the following:
Patient care rounding with multidisciplinary teams Provide comprehensive pharmaceutical care including: - Effective clinical interventions and medication order clarifications - Pharmacokinetic consultations - Promote, evaluate, and monitor appropriate, rational drug therapy use - Prevent, detect, monitor, document, and report adverse drug reactions and medication errors - Contribute to discharge/transfer planning relative to drug therapy when necessary - Patient/family teaching and inservice education - Drug distribution support - Participate in drug therapy management of medical emergencies when necessary - Opportunities are available for pharmacy representation in interdisciplinary committees - Teaching opportunities are also available through our affiliations.
Minimum Qualifications: Required or desired credentials or experience of applicants: The successful candidate will possess a Pharm.D. degree from an accredited School of Pharmacy. Hospital experience preferred but not required.
Degrees, Licensures, Certifications: North Carolina Pharmacist License *This position is with one of the largest teaching hospitals in the US.
