Primary Functions/Responsibilities:
We currently have an opportunity for a pharmacy professional to join our Georgia Regions Chronic Kidney Disease Service. This individual will work closely with Kaiser Permanente providers to promote cost-effective prescribing for members. This position will also partner with physicians to optimize the medication therapies of chronic disease states. Additional responsibilities include developing and presenting educational programs to patients, practitioners and medical office staff and precepting students and residents.

Required Competencies and Skills:
- Excellent written and verbal presentation.
- Excellent interpersonal skills.
- Experience in delivering presentations to physicians and working with medical staff required.
- Proficient in Word, Excel, PowerPoint and Access.
- Strong decision making and problem solving skills.

Required Education, Experience and Training:
- Doctor of Pharmacy degree required.
- Residency and/or fellowship in general clinical pharmacy (e.g. Pharmacy Practice, Managed Care Pharmacy Practice, Primary Care, Ambulatory Care) or appropriate specialty area depending on the scope of the service, or is BCPS certified.
- Current GA pharmacy license.
- Expert knowledge of drug therapy management of primary care (e.g. hypertension, diabetes, asthma, CAD, anemia management).
- Clinical: Measurement of blood pressure, use of blood glucose monitor, use of inhalers, peak flow meters and Lovenox patient teaching.
- Complete a Customer Service Assessment*.

Preferred Education, Experience and Training:
- Previous experience in anticoagulation monitoring, cholesterol management and/or appropriate primary care area preferred (e.g. chronic conditions experience for Medication Management CPS)
- Customer service aptitude demonstrated through Customer Service Assessment*.
