You will practice as a palliative care clinician for hospice patients, providing end-of-life pain and symptom management. In this vital role, you will recommend appropriate pain and symptom management therapies based on patient specific information, working with nurses to create cost-effective medication care plans. An important part of your work focuses on medication misadventure prevention by providing education to nurses and caregivers.
Requirements for this position include a PharmD from an accredited school of pharmacy and an active pharmacy license in good standing in Pennsylvania (AZ or TN).
