Job Description:

Minimum of 10 hrs/wk and maximum of 31 hrs/wk Responsibilities: Advancing patient care and documenting interventions for outcomes measures; recommending appropriate pain and symptom management therapies based on patient specific information; providing real-time collaboration with nurses to create cost-effective medication care plans; focusing on medication misadventure prevention; providing education. Research and publication opportunities exist for interested clinicians. Candidate must possess excellent communication/interpersonal skills.

Experience and Skills:

Qualifications:Qualifications: • PharmD degree from accredited school of pharmacy required • Possess conceptual understanding of pharmaceutical care with commitment to patient care • Honed clinical assessment skills • Excellent communication skills • Proficiency in computer applications • Problem-solving skills • Self motivated, self directed; team player • Ability to follow through on tasks assigned excelleRx is an equal opportunity employer, offering a competitive wage and benefits package including 401K and stock options.
